1.       Are the source’s name and other identifiers redacted from the requisition and results? Create unique identifier for both source and employee

2.       If the source name is associated with the order and result, are the orders included in the medical record or stored elsewhere? Not stored in medical record

3.       How are the results of the tests shared with the source? Source or parent calls Employee Health with unique identifier number to get results
