We have all respirator users do the OSHA short form every 5 years and fill out an abbreviated form the years between (any health changes, changes in workplace requirements, etc.).  N95 fit testing is done yearly.

I interpret OSHA’s regs to mean  anyone using a respirator   (N 95 or PAPR) must fill out the medical evaluation. This is not an annual requirement though.  The only annual requirement is fit testing for the N 95.

We use Max Air System Powereed Air Purifying Respirators at our facility. Recently it was brought to my attention that these can not be used in the operating room or the MRI dept. Therefore we started doing fit testing for these departments so they can use the N95 mask. We do  the OSHA Respirator Medical Evaluation Questionnaire for all employees however I wanted to know do other facilities just to the questionairre for only employees who use the N95 or for all employees?
