As part of our surveillance process for hazardous drugs, we plan to send a letter to the home of any terminated employee who worked in an area with hazardous drugs.  This letter will encourage the employee to return to our clinic for testing.  We are working with departmental managers to make this part of the exit process; however, the letter is means of catching individuals who leave the facility without completing testing. Do any of you have a similar process in place?  If so, would you mind sharing the documentation you provide to the terminated employee?
