Question to those who perform DOT physicals:
If a 30 day temporary card was issued to a driver for them to get some medical clearance prior to 4/20/16 on the older version of DOT certificate/long form, and they bring an acceptable clearance now, are you issuing the new card/certificate based on physical done prior to 4/20 on the older long form? Or doing an entire new physical on the new form.
