When I worked at a hospital in Charlotte North Carolina in the early 1990’s we would have all our exposure and source patients come in with a special number and the males were “Michael Jackson” and the females were “Shirley Maclaine” – When we worked in the lab we just knew it was a privacy thing, but now that I think about it, it might have been all Medical Records and L&I for all I know..

2. If the source name is associated with the order and result, are the orders included in the medical record or stored elsewhere? Not stored in medical record

3. How are the results of the tests shared with the source? Source or parent calls Employee Health with unique identifier number to get results
