We implemented a voluntary “population health” program for our employees/spouses in 2014. It is based on care management of chronic conditions and not a wellness program. We incentivize employees with health insurance discounts, free medications
and other value based incentives. We have 2 goals, improve health outcomes and reduce costs. It has been successful except for continued emergency room utilization for non-emergent reasons. My goal for the original post is every 2 to 3 years, my organization
likes to “go to market” to see if anyone is doing something more innovative or creative.

I recently did a literature review on this topic and very little data exists to support this type of activity and I found nothing that demonstrated sustained levels of wellness. Our own (admittedly limited) program
has not shown improvements in population health. Moreover, we found that since we can only offer the program ‘voluntarily’ often the people who volunteer to participate are already people with high levels of wellness indicators.
It could be very risky, given the need to keep programs voluntary in employment situations and keep medical information protected, to implement an intensive program with enough incentive to stimulate change.
I would be interested to know if anyone else has a more optimistic view!

Has anyone had success with any population health/wellness/disease management programs specifically for your employee population? If so, would you be willing to talk offline about your program and/or share any vendors you are using. We
are interested in care management programs for employees with chronic conditions (diabetes, hypertension etc.)
