Lisa,
Thanks for putting together the form for us to save a copy and use. It
saved me a bunch of time since I didn't have to copy and paste
titles/authors to make my own Google form this year!
Once I saved my copy, I did change the question format from "checkboxes" to
"Choose from a list." This way students can only choose ONE book.
(Checkboxes allows for students to check off more than one title and that
would throw off voting results.) Also, from using a Goog form in the past,
I've seen common mistakes that can happen. I make all questions "required."
I also added a question for students' names. I don't share the names with
anyone, but one year I had a student submit like ten forms/votes (by
mistake, honest) for one book. Because I had students submit name with
form, I was able to talk to the student, find out it was a mistake and
adjust our voting results to reflect the book he chose once (instead of ten
times!)
