[CASL-L] Help creating surveys for faculty & students

Hi, 
I have the task of creating a survey to "evaluate the library media program and its services and to provide a forum for faculty and students' input and suggestions." I plan to create two surveys for my grades 1-6 suburban school, one for faculty and one for students. I plan on using Survey Monkey and I have lots of ideas of what I'd like to ask but before I dive in, I thought I'd tap into the collective experience and wisdom out there. Have you done a survey in the past? For faculty or students or both? Would you be willing to share the questions and how you structured it? Any other advice? Thanks in advance!
