Our public library provided me with a simple application that I send to the public library through the interschool mail. They process the application and send me the cards. The process does take a couple of days.
I discuss the procedure during my freshman library orientation.

We just brought our public librarians in to meet the freshmen. Every student either brought in a library card or completed a library card application form. Our public librarians agreed that attendance at our local, public high school constituted proof of residence so students didn't need to present any additional information.

Once they all had a card, I had students create a google drive document where they recorded their library card number so they couldn't lose it and would always have access at home.
