Hope all is well!

Here is the Google form I use.

I do not have them sign out right now. It's hard enough getting them all to
sign in. I have the three computers closest to the door set up with the
Google form. I use a generic student account and leave it logged in. There
is not enough time to have them all sign into their Google accounts.

I'm looking into subscribing to Library Trac. They were willing to sign a
student privacy agreement with my district. It sounds like it would work
out really well.
