[CASL-L] Collaboration/Research Form?

Good Morning:

Thanks to everyone who shared their ideas and offered help and suggestions.
I continued to work with department leaders on developing a simple
electronic method to initiate collaboration; attached is a PDF of the
Google Form I created. This form is obviously very specific to my situation
but is working well so far. I have already received several requests from
teachers that have previously never visited the library with a class.
