[CASL-L] summer reading logs

We created a really simple Google form for their responses (we didn't ask for a log, but rather two quick responses about books they chose to read; you could set it up however you wanted, though, depending on what you're asking the kids to do). We posted the link on the school website and teachers' Google Classrooms as well as through the public library. I was able to give a public librarian access to the responses spreadsheet so students could earn credit towards their summer reading program through the school's form. One of the things students filled out was the name of their English teacher, which made sorting/ grading the responses easy for the teachers in the fall. We surveyed kids ahead of time about how they wanted to report out on their summer reading, and a Google form was by far their top option. So, I worked with the English department, students, and the public library on creating this, and it went very well. Win/ win/ win!

Does anyone have an efficient way of having students log their summer reading that you can share? Pages read, books read, etc., either something you came up with or an app that you think works well?
