Hello,

Does anyone have a form that they have created/found to give to individuals who donate materials to your makerspaces?  The materials often go to the office and I don't know who donated them.  I would like to have a form that the person who takes them (usually the administrative assistant) can have the donor fill out.  

The purpose would be to have a record of all donations, as well as info for where to send a thank you note/letter/email!
