Thank you everyone for your help and feedback with the question I posed last Friday. Many of you recommended Google Forms for my electronic sign-up. After some investigating and exploration, based on your recommendations, I found exactly what I needed when enabling the Form Limiter add-on and the Choice Eliminator 2 add-on to Google Forms.
