Hello Kara,

We use Destiny to track fines.  We charge $40 per year (covers summer usage as well) for 2 small claims (broken screen, lost charger, keyboard breakage) or one large claim which is a device replacement due to things like water damage or mom ran over it with her vehicle.  We also charge $25 for charger replacement for those students that do not have insurance, $50 for screen replacement, etc.  If students purchase insurance each year and do not have a claim against it, they get a $10 break the next year, so seniors could potentially pay $10 for their cb insurance senior year.  Students entering school late (usually by spring time), we charge $20 for insurance for the remainder of the year and coverage over the summer. 

We keep a spreadsheet of all students that purchase insurance each year to help us keep track.  Students who are unable to pay or have financial difficulty can see their admins. for help (to lower the cost of the fee, waive the fee, give them more time to pay, etc.)   Hope this helps.  If you need more info. on pricing (what we charge for other things), let me know.
