[MCOH-EH] Return to Work Form

Thank you for the valuable inputs so far. I like the idea of cognitive clearance on some cases, do you send to psych or who exactly?

By the way, Lisa do you happen to work in Valley Medical Center in Renton, WA? I worked there during my training with Dr. Karen Nelson. Is she still there?
