[MCOH-EH] Screening Transfers

Screening transfers in a large health care system is a challenge, but is
important for the reason mentioned below, as well as many others.  At my
last place of employment, Employee Health received notification from
Recruitment of all transfers from one job to a different job category (i.e.
not, for example, for a nurse transferring from the night shift to the day
shift or for a secretary in one department going to be a secretary in
another department), and we performed a chart review to make sure their
immunizations and TST were up to date, they had been offered the Hep B
vaccine if appropriate, and they didn't have any ongoing job restrictions or
health problems we were aware of that might be problematic in the new
position.  If any issues were found, then we would require an EHS visit
before the transfer could be finalized.  Otherwise, we would clear them
based on the paper review.  With the typical 2 week notice required to
transfer, that usually gave us enough time to accomplish this without
causing delays. We screened about 1,000 transfers a year this way. It was a
lot of work, but I think it was very worthwhile.  It also served as a nice
ongoing quality check on our immunization and TST programs.

we do vaccinate or restrict from patient area work.  It is amazing how often
after that people are able to find the documentation or accept the vaccine.
Our issue is that if you are in a non patient area, later you can transfer
to a patient area and we have no system to catch these.

For those hospitals that require proof either by vaccine hx or titer, if
someone is not immune do you then require the vaccine?

We follow CDC recommendations, but if someone is not immune we don't make
getting vaccinated mandatory.

We do counsel that they may not be able to work with certain patients or in
certain areas, such as mother/baby but we can't force someone to be
vaccinated except if they work on mother/baby as Mass law requires it.

Supervisors are notified of any staff who are not immune to one of the 4.

Wondering if anyone makes them sign a declination with consequences spelled
out?

There is no difference in our requirements between types of incoming
employees. MDs, NPs, PAs, Nurses, non-clinical staff all require proof of
MMRV immunity on hire. Older employees were presumably cleared when they
started but we do not have that info in our EHS computer system. When we see
longer-standing employees for clearance (recently Ebola teams) or
surveillance we try to update their records and will offer either titers or
vaccines if needed but there is no requirement that they do so.
