[MCOH-EH] Return to Work Form

We use these to return our employees to work for any non-work related injury/illness reason…..after fractures, cardiac caths/stress tests, maternity leave, surgeries, etc. For general illnesses that could in no way have restrictions such as lifting, etc (GI illness, strep throat, etc, we only require a MD note). If there are any restrictions written, we evaluate on an individual basis for reasonable accommodations. If an employee is cleared but we have concerns backed by objective data (ie. employee tells others they can’t lift or states their back is hurting, etc), on occasion, we may then send the employee for a Fit for Duty (our cost).

Can you identify the goal of your RTW form.   Is this after employee injury or infectious illness or fitness for duty or ADA?   Or personal illness?

We are developing a new form for the university employees and I will like to look at multiple formats before finalizing our own. This is a major science research university however employees' backgrounds range from teaching academic staff to adminsitrative staff to postdoc and postgraduate students.
