[MCOH-EH] preplacement evaluation at Mayo

We appreciate the reference to the Mayo system.  Bob speaks truth.  We have been using our current system for 11 years, and it seems to work well.  We published results of a pilot study in JOEM in late 2011, showing little to no difference in discrimination compared to the old, labor-intensive system.  The system is 99% RN-based with protocols and rare need for MD input.
I downloaded the 3 forms we were using to the Idea Marketplace in 2011, and they are in the Work Disability section of the Marketplace, almost at the end of the list.  They have morphed over time but are pretty much the same.
