The Employee Health Nurse and one other RN who has been trained do the exam which consists of:

Medical History Questionnaire. This questionnaire also has a section asking for the dates and types of the past three employers. If any gaps in time are  noted, the new hire candidate is asked to explain why.
Color blind test
Immunization review and drawing of titers, as needed
TB questionnaire looking for active symptoms of TB and drawing of Quantiferon TB test.
N95 fit testing
Blood borne pathogen education
TB education
Urine drug screen.
Review of job description and job physical analysis. New hire candidate is asked if they have any questions or concerns and asked if they feel they are able to perform the essential functions of the job. If they do not feel they can or the nurse feels there might be some indication they couldn't (like recent history of a back injury), they are sent for a fitness for duty exam by their provider at their own expense. If they do not have a provider we will help find one or refer them to an occupational health clinic that does these exams.

Hello all
As with many of our health care systems, we are asked to use resources wisely. I am interested in what you EHS does for pre-placement evaluations. We have traditionally had NP's do the exams. We have the RN;s do the paper clearances for certain positions, with referral to the NP for red flags.

We are thinking about having RN's perform the basic exam and defer to the NP or MD when there are questions.

Correct. There may be some variation between Mayo sites, but at Mayo in Arizona we do not have an NP. The RNs do the preplacement eval including some basic labs, vision, hearing, health and immunization questionnaire review, TB and fit testing (where needed) and drug testing. A physician is consulted only in areas of concern or for a positive drug test. The physician also does a paper review and signs off on the nurse's eval.
