[MCOH-EH] PAPR usage

1.) The AAOHN Webinar-Building a Best Practice Respiratory Protection Program using Case Studies (originally aired 2/3/15, but is now archived for viewing) answers some of your questions. They stated the OSHA questionnaire had to be done only once; there  was nothing by OSHA saying it had to be done yearly. We do, however, do ours annually as I want the latest information when I am determining medical clearance. We use the same form for N95 and PAPR. It has 15 questions. If we are screening for hazmat respirator use, there is an additional 7 questions.
2.) Unsure about height/weight/age. Our questions come directly from the OSHA form and it might be there requirement?
3.) Training is annual with a power point and test. One time hands-on competency is done within 30 days of hire.
4.) We train the following departments in PAPR use: respiratory therapy, emergency department, and security, and anyone who cannot wear a N95.
5.) PAPRs cannot be used in the OR as they can contaminate the sterile field. CDC TB Guidelines state "When surgical procedures (or other procedures that require a sterile field) are performed on patients with suspected or confirmed infectious TB, respiratory protection should be worn by HCWs to protect the sterile field from the respiratory secretions of HCWs and to protect HCWs from the infectious droplet nuclei generated from the patient. When selecting respiratory protection, do not use valved or positive-pressure respirators, because they do not protect the sterile field. A respirator with a valveless filtering facepiece (e.g., N95 disposable respirator) should be used."

                We are  switching to PAPRs over N95 masks and would like some of your expert guidance.

1)      How often does the OSHA questionnaire need to be done and do you use the long or short form?

2)      We have a lot of staff that complain about the need for height/weight/age.  Is that necessary to ask?

3)      How often do you re-train on the PAPR use?

4)      What job categories do you test?

5)      Do you use PAPRs in the OR?
Any information would be very helpful.  Thank you.

The Employee Health Nurse and one other RN who has been trained do the exam which consists of:

Medical History Questionnaire. This questionnaire also has a section asking for the dates and types of the past three employers. If any gaps in time are  noted, the new hire candidate is asked to explain why.
Color blind test
Immunization review and drawing of titers, as needed TB questionnaire looking for active symptoms of TB and drawing of Quantiferon TB test.
N95 fit testing
Blood borne pathogen education
TB education
Urine drug screen.
Review of job description and job physical analysis. New hire candidate is asked if they have any questions or concerns and asked if they feel they are able to perform the essential functions of the job. If they do not feel they can or the nurse feels there might be some indication they couldn't (like recent history of a back injury), they are sent for a fitness for duty exam by their provider at their own expense. If they do not have a provider we will help find one or refer them to an occupational health clinic that does these exams.

As with many of our health care systems, we are asked to use resources wisely. I am interested in what you EHS does for pre-placement evaluations. We have traditionally had NP's do the exams. We have the RN;s do the paper clearances for certain positions, with referral to the NP for red flags.

We are thinking about having RN's perform the basic exam and defer to the NP or MD when there are questions.

Correct. There may be some variation between Mayo sites, but at Mayo in Arizona we do not have an NP. The RNs do the preplacement eval including some basic labs, vision, hearing, health and immunization questionnaire review, TB and fit testing (where needed) and drug testing. A physician is consulted only in areas of concern or for a positive drug test. The physician also does a paper review and signs off on the nurse's eval.

The RNs in the department review the questionnaires.
They consult with the NP or MD as needed on the "yes" questions to determine whether the person should be cleared.

I am wondering how you handle annual ppd questionnaires for those who have had a +ppd or IGRA tuberculosis screening.  We perform our annual questionnaires on 5/1 of each year.  My question is if you hired someone in January, February, March or April and they filled out the questionnaire on hire, would you have them make out a new questionnaire with the annual May surveillance or would you put their next due date as 5/1 of the following year (example someone hired 2/1/15-next due date for questionnaire 5/1/15 or 5/1/16?) We do the same here.  Do you require a past negative chest x-ray on start or do you do a chest x-ray on start and then do the tb f/u annually after that?

If they have had one in the past year, we accept that. If not, we complete at time of hire and then do an annual questionnaire.
