1.       How many employees do you have in the organization? 
	Approximately 1300.


2.       How many employees do you have in Employee Health and what types (full time, Part-time, PRN?) and functions (Basic EH, Worker comp, Wellness, Exposures, onsite acute care) ?
	One full time RN-Basic EH, ergonomic evaluations, fit testing, new hire exams 1 day a week, fitness for duty letters and follow-up, vaccinations, manages fall flu campaign, sits on various safety committees, participates in general hospital orientation, f/u blood borne pathogen exposures, back up resource for Infection Prevention Nurse, consultant for workers compensation coordinator, back up for workers compensation coordinator.
	One RN 1/2 day a week-completes and processed new hire charts, gives vaccinations, fit tests, provides vacation relief.
	One full time Workers Compensation Coordinator, also helps manage Healthstream education program for organization, presenter for general hospital orientation, sits on safety committees.

3.       What functions do you perform in Employee Health (Pre-placement, annual assessment, immunizations, sick visitsetc?

4.       Do you have staff dedicated to Wellness, how many?
We have one full time Wellness Coordinator that works separately, but does collaborate with Employee Health

5.       What services does your Wellness Program provide?
Nutrition tips, exercise tips, cholesterol screenings, tobacco cessation, Employee assistance program, life balance

6.       How many staff for Workers compensation?
One.

7.       Do you have a Workers Comp physician/provider in house or is that outsourced?
Outsourced.

8.       Who tracks exposures and injury logs?
Worker's comp coordinator

9.       Do you provide an acute care or onsite clinic? No.         If so- how many employees?                  What is the charge?


10.   Do you complete IGRA testing (Quantiferon or Tspot) for TB surveillance? Yes, upon hire and post-exposure only.

New Hire immunizations, tb testing, drug screening, surveillance reviews etc...  This is all new hire stuff Annual Tb testing Annual flu shots mandatory Follow up vaccines Annual surveillance questionnaires ...review and follow up for 15,000 Tb questionnaires, 150 vet services questionnaires, 8,000 resp fit questionnaires, 150 hearing questionnaires...
Manage the badging portal for some 8,000 non cchmc vendors students contractors etc....
Manage all exposures and infectious disease issues DOT new hire and annual and random testing Whatever else comes up

1 Workers Comp person who also does ergo evals

2 coordinators who do administrative work to free up nurses to do what they need to do... manage programs like safety college, flu, integrity, tb campaigns, etc...

Responsible for the overall Employee Safety initiative

Benefits does wellness

Onsite clinic, ...not sure

I have a PA that does injury management

8.       Who tracks exposures and injury logs?
WC person

9.       Do you provide an acute care or onsite clinic?          If so- how many employees?                  What is the charge?
Benefits does

10.   Do you complete IGRA testing (Quantiferon or Tspot) for TB surveillance?
Quanteferon on new hires.... Skin test and QFT for annuals...depends on location
