[MCOH-EH] Occupational/employee health records AND other medical records

Just curious on whether occ med/ urgent care practices keep all records in 
the same patient chart or have different charts for occ health and urgent 
care. 
Moreover, those who have EMR, is there any "separation" between the two 
records. Thanks.
