We are a rural California hospital and follow the guidelines/recommendations from the California Department of Public Health. These are based off of the ACIP guidelines for healthcare personnel.

We are a small community hospital here in middle Tennessee.  The new hire process: TB test, if positive then chest x-ray and referral to public health if 10 or >; Flu immunization, rubella, rubeola titers; HEP B offered if indicated by checking one of the boxes: previously vaccinated, Request; Decline; Tdap is offered to any working with children or high risk areas: CCU, ER, OR, Peds, Peds ER.

I am interested in learning what vaccines and other post-offer (pre-employment) tests/titers are required at  other healthcare organizations and hospitals.    Currently, we require:

Tuberculosis survey and titer
Flu vaccinations
Measles, Mumps and Rubella (MMR) titers
Varicella titer (Unless there is history of Chickenpox) Hepatitis panel TDaP (Tetanus, Diptheria & Pertussis) vaccine - may be declined by employee

Annual requirements:
TB Survey
Flu vaccine

Actually 26% negative for mumps alone.
Rate is much greater if you include indeterminants.

Do you have the reference for the 20% negative/indeterminate statistic you mention below? We are currently reviewing our processes in this regard.  Also, can I ask whether your institution has looked at making Tdap mandatory as a one time adult dose, if no documentation provided?   Thank you in advance for your expertise and response.

We follow CDC recommendations and require proof of immunity to measles, mumps and rubella.

Proof of immunity for measles and mumps can be met by date of birth before 1957.

We accept documentation of vaccine, or positive immunity tests. If they do not have proof of immunity for measles and mumps, we offer vaccination.
We do not perform screening for immunity in lieu of vaccination, the exemption being people born prior to 1957 who only need rubella vaccination.

Individuals may elect to have lab testing done (at their own expense) with our warning that approximately 20% of adults will have negative or indeterminate results on at least 1 of the 3 tests so would need an MMR in that event.

Post-vaccination serology is not recommended.
