[MCOH-EH] Respiratory Fit Testing

After the initial medical evaluation, we use the following 3 questions in an online questionnaire. A person who answers yes to any one is automatically referred to the Occ Med Service.

Since your most recent medical evaluation for respirator use, has your health worsened in any way or have you developed a medical condition that may interfere with your ability to wear the previously fit tested respirator?

Have you been told by a health care professional, your supervisor, or the respirator program administrator that you should be medically reevaluated?

Has there been a change in workplace conditions, for example, physical work effort, protective clothing or temperature, that has resulted in a substantial increase in the physical burden on you?

No we use a shorter version that basically asks for updated information or changes

No, only at time of hire, then an abridged questionnaire as part of their annual health screening.
