[MCOH-EH] PAPR policy

I am interested in healthcare organizations that utilize PAPRs .

Here are my questions:

1. Is PAPR use  across the board for all employees?
2. If you use PAPRs and N 95 masks for select groups, what is your 
criteria for using the N 95?
3. Who bears the cost of the PAPRs?
4. Do you have a policy that you would be willing to share?
