Yes, we have been doing randoms for 2 years now.  We notified all employees 6 months in advance via letter to home mailbox that we would begin this and the reasons why.

We have a house wide pool that our meditrax software picks names from.  We have 3,000 employees and complete 17 randoms/month.  We have high risk areas (ED, ICU, others with increased autonomy, access, etc) screened more often than those in billing offices, without access to narcotics.  We have seen about a 1% catch rate, usually with illegal substances.  Once with alcohol in the 2 year period.

Janice moved from USAMRIID a few years ago but is a great resource.  While the MMWR is helpful and at Eagleson June conference a couple years ago we had CDC there to present the MMWR piece and a number of work groups to review it there, the surveillance part leave lots of questions and Roger at UC Davis has had some cases of late and so has de facto become very deep expert in this area.

Janice Ruznack (spelling?) at USAMRIID is a good resource. I believe they sometime will even facilitate providing the vaccine under an IND protocol if the work is high risk.
