[MCOH-EH] Wellness/Disease Management

Has anyone had success with any population health/wellness/disease management programs specifically for your employee population? If so, would you be willing to talk offline about your program and/or share any vendors you are using. We are interested in care management programs for employees with chronic conditions (diabetes, hypertension etc.)
