We currently use the 3M1860S, 3M1860 Regular, and Kimberly Clark duckbill
(small) and have the same issue. About 1/3 of our staff can not fit to any
of the N95 respirators we offer due to facial changes as in pregnancy,
facial hair or just not a satisfactory fit and are assigned a PAPR. Our
N95s are needed infrequently at our facilities, which also causes the
problem of improper mask placement despite annual training. We are in the
process of looking at replacing our current PAPR with a MaxAir CAPR. This
is a more comfortable, faster donning/doffing, more patient friendly
"PAPR." The added benefits would be increased protective factor and an
overall cost savings by no longer having to perform fit testing annually.
Unfortunately, we would need to continue fit testing OR staff, but I
believe that Syntech is in the process of FDA/NIOSH approval of the CAPR
for use in sterile environments.

Hope this helps...my big issue with N95 respirators is the less than
optimal protection they provide in light of increased globalization and
novel diseases.
