Good morning Cathy,

QYK (formerly Vac-Seen) and other scanner systems are unnecessary with
MediTrax. Rather than requiring users to pre-load consent forms into a
device with a small screen, we have implemented a process for conducting
paperless flu clinics which requires simply selecting each employee from
a picklist. This enables rapid documentation of immunizations and
declinations for non-employees (volunteers, contractors, students,
/etc./) who may not have a scannable ID badge.

The flu-clinic data entry screen allows documentation of immunization by
various individuals, as well as full-screen display of
questionnaire/consent forms completed in advance via the Employee Portal
(users preferred this to viewing questionnaires in small print on a
palm-size screen). Free-text notes may be documented for any
immunization or declination, and the individual's immunization history
may be displayed with one mouse click.

The values shown in this screen shot are all pre-populated. Total
computer time required for each immunization is usually between 15 and
30 seconds. There is no need for a separate process to upload the
collected data into MediTrax.

MediTrax includes similar paperless no-scanner-required documentation
processes for group TB testing and group respirator fit testing.

Regards,

Jodi Powell
