We use an iPad app for OHM electronic medical record and the consent as well as the vaccination, including who gave it, goes right into the EMR at the time of the vaccination. Of course, includes date, lot #, exp date, etc.
Works well.

We do not have a separate consent for the flu vaccine as they are given the VIS sheet to read and they have to read at least #3.  Most of them do not keep the sheet so we don't have to print many, maybe 100 if that and we have over 3000 employees.
We document in Medgate and it has all of the information-from name of vaccine,manufacturer,lot number,expiration date,name and credentials of who gave it.  We data enter the information once and it also has a copy function that we use for our mass clinics.  We also enter the VIS in the data base so it automatically records this with each vaccine when given.  Many need a copy for school or another facility and because we have the Portal self-scheduling module of Medgate they can obtain their records and print them from work or home. Our goal was to decrease the amount of paper that was flying around in the office and also clerical time my licensed staff was doing.

We use the Qyk scanners as well. The scanners create a PDF that serves as the consent and administration record so folks can give it to other organizations (schools, other employers, etc.). The elimination of physical paper has been such a great change!

We use QYK scanners for flu vaccines, scan the badge and it enters in our database.  We ask a few questions on the scanners to ensure there are no contraindications but have no paper consents.   We use electronic consents for our other vaccinations and the employee signs using a electronic signature pad.

We have a one time consent for flu.   The employee does not sign it again.   We just update any new medical concerns each year.

I have consents for all my vaccines, and have since created a log in sheet when they decline besides just the Hepatitis B, so I can show I offered the Tdap or MMR booster if they were low and they choose not to get it. That way, if we have an outbreak and they are quarantined to home for 21 days since they don?t have immunity, I can at least prove it was offered to them. the declination states for certain outbreaks what the scenario may be if they have no proof of immunity.

Hello all
In the past we have had employees sign a consent when we administer the flu vaccine. We do not do this for any other vaccine because the CDC states if the VIS is provided and documented that it was given, no written consent is needed. in our sate of Massachusetts, written consents are not mandatory.

This year we have an electronic tracking of giving an employee the flu vaccine. It automatically goes into our data base and the employee and their manager receive confirmation of the vaccine and the pertinent information. It also goes into our EMR and then transmitted to the MIIS for Massachusetts DPH.

Our leadership is questioning  the elimination of the consent form(this would be about 12,500 pieces of paper). Their question is "what are other hospitals doing?"
