Good Afternoon,


1.       Do you require  new hires, contractors, students, volunteers to complete a health history form? If so, could you share  the form?  We do not do this

2.       Do you ( NP, PA, Physician) perform a mini-physical on new hires, contractors, students, and volunteers? No... we only do a physical if it is related to a specific identified hazard or regulatory requirement
