It's unlikely that CDC/ACIP will change the Tdap recommendation for HCWs because protection from the current acellular pertussis vaccines wanes within several years. I'm attaching the presentation that was given at a 2014 ACIP meeting, which explains this decision. That said, hospitals may revaccinate employees working in high-risk settings like NICUs if they choose too.

Since we are still seeing Pertussis I have a hard time not repeating the Tdap every 10 years.  I am hoping the CDC will change the recommendation to every 10 years.  So far we are giving a 2nd Tdap.

If they already had Tdap once as an adult and they can show us documentation, we go to Td.  If no documentation of Tdap as an adult, we administer this.  We also give Tdap to women in their last trimester of pregnancy.

When an employee (new or existing) had a TdaP 10+ years ago, do you give a Td or another TdaP?  If just a Td, what about those in obstetrics or the ED?  I know CDC says Td, but I'm seeing a lot of family practice providers giving a 2nd TdaP.  Thank you for any guidance you can provide.
