[MCOH-EH] patient handling risk assessments

We are involved in the SPS Behavioral Injury Events cohort and are looking for other childrens hospitals that have an intake questionnaire for all patients that asks about behavioral violence events prior to this admission.
Looking for the initial questionnaire, which may be brief and then if YES to a prior behavioral issue, I would like to see the next checklist used to determine:
What kind of behavioral violence risk does the child have
What are the soothing measures/trigger points
What PPE is determined to be used on the patient for employee safety
What environmental changes should happen to provide safety to the patient (ex. Remove cords, sharp objects, televisions)
