Good morning,
Our current process is the same for all employees throughout our healthcare system, which is made up of 3 hospitals and several clinics & OP settings.
Employees notify their immediate supervisor and house supervisor, then complete an injury form or exposure packet. After completing paperwork, packet and/or forms are brought to Employee Health (EH) for evaluation. If further work up of the injury is required (Xrays/sutures..) they are referred to our MD at the Occupational Health Clinic. My department has 3 NPs on staff who see and treat BBF exposures, with the exception eyes and women who are pregnant. After initial treatment has started, employees are then referred to ID for follow up and return to EH for scheduled required labs. All chemical exposures and exposures of the eyes and women who are pregnant are referred to Occ Med.
We are in the process of moving the location of the above forms to MIDAS, to allow filing online before reporting to EH.

All incident injuries near misses exposures or safety concerns get reported to one Employee Health 24/7 hotline The information is recorded into our database that creates the OSHA log

OSEH follows up on safety issues.

Yes we have all accidents and injuries come through Employee Occupational Health.  We have an on call service for off hours when we are not in that sends us a report the following day and then we call them back to follow up on it.  In my previous position at another hospital they could report on line.
