Tim statement is correct. We screen people w this questionnaire during their annual TB, mask fit etc time. Hope it helps. It could easily go electronic in the ELS but we just do it annually w their TB. Hope it helps. Feel free it use just change up the headings, etc.

Can't quote chapter and verse, but they do not require an annual fit test;  they do require annual training; this can be done through an online educational platform so doesn't require a visit/occ health employee bandwidth.
