Hello,

My employer is looking at providing an onsite primary care clinic for our employees and their adult dependents in addition to our current occupational health offerings.  We are in the process of gathering data on these clinics and would appreciate your input on the following questions (you can only answer questions you are knowledgeable about or feel comfortable answering):

  1.  What is your organizations total health care spend?     We are self insured with   8500 team members,  we have around 6500 covered team members and a little over 14,000 covered lifes (includes covered spouses/children)   Not sure if I can release our total healthcare spend publicly.
  2.  Do you offer a patient centered medical home (PCMH) style clinic or focus on subsets such as preventative care, acute care, chronic condition management, or occupational medicine?  We have 17 primary care clinics for both team members and community.   Our Team Member Health clinic (aka Employee Health) has two nurse practitioners who provide acute care and occupational health services for injured team members.   We have two disease management nurses under the Team Member Health division  who coordinate our "HALO (Healthy Actions for Lifelong Outcomes)" program for team members with diabetes, obesity/metabolic syndrome and hypertension.  We are hiring a wellness dietician and also have two other wellness educators who work with our team members to improve their wellbeing.  We have a WELL weight program in collaboration with our Bariatric Center.  We also have a Pathway to Wellness program starting soon which will combine metabolic analyzer,  Tanita scale, waist measure, biometrics and require consultation with our DM nurses and dietician to earn WELL Points on our wellness platform.
  3.  What do you spend on your primary care clinics?  Not sure...we are trying to get an onsite primary care clinic just for chronic disease mgt.
  4.  Do you use a vendor partner or provide services internally?  Provde services internally
  5.  How many employees and adult dependents would you estimate have access to these services and also work or live in the serviceable area out of your total population?  14,000
  6.  What percentage of these employees and adult dependents have used your primary care services?   60%
  7.  Have these services increased member utilization and or the overall cost of primary care?  NO
  8.  Have these services had a positive impact on the following:

     *   Helping better manage your health care cost (decreasing your spend/trend)  Yes,  we use a data mining vendor that indicates those in our programs cost , on average, $1400 less than those who do not participate
     *   Management of chronic conditions  We have great data supporting our program.s
     *   Preventing or lowering overall group health risk
     *   Productivity  Just anecdotally
     *   Organizational culture  WE have great participation in our wellness platform and tie all our internal programs into the Virgin Pulse reward system we are going live with in July.   Prior to VP,  we used another platform with between 60% and 70% participation per year and great results.!
     *   Retention
     *   Recruiting
     *   Lowering health care cost in other areas (for example, in ER over utilization, unnecessary speciality costs, imaging or helping manage pharmacy costs)  Still working on developing data for this.
     *   Anything else?  We just changed TPA medical plans from UMR to BCBS so need to wait until our data mining vendor can download claims data from BCBS before we can analyze costs.  Our DM nurses and HRIS analyst have access to individual claims as part of their job duties and we can pull reports and have targeted marketing and communication to team members/dependents with specific conditions.   We are working on metrics with our data mining vendor to compare claims costs  for participants in our programs compared to those who are not.

10) On a scale of 1-10 (10 being completely satisfied), how satisfied are you with the individuals or vendors partner managing your clinic?   We manage

11) Anything you would like to improve or would have done differently in managing your clinic(s)?    Have onsite main campus chronic disease mgt clinic for team members and spouses in collaboration with Team Member acute care clinic.
