Consider getting a HCV RNA by PCR.  That way, you can refer to hepatology to follow and, potentially treat, early HCV infection, if and infection is acquired and if it does not resolve on its own.  Fortunately, the new pan genotypic drugs have taken some of the fear out of this exposure.
Of course, you also may have to deal with a potentially infectious healthcare worker but that is usually not an issue unless he/she is performing highly invasive procedures.

I’d like to hear how others are managing post-exposure with a source patient who shows REACTIVE HCV antibody and HCV RNA DETECT/QUANT 200,000+.  Do you typically perform a baseline panel on employee with a repeat in 6 weeks, 4 months, and 6 months.  Other than counseling the patient to practice safe sex and report any symptoms, is there anything else you would recommend?
