[MCOH-EH] policy for mishandling of medications

I'm looking for information on how other facilities manage missing medications in the outpatient setting like home health or hospice care. Do you have a policy or protocol on how employees are processed when a medication is missing or when a patient of family member makes an accusation that some medications are missing. I'd like to hear about any kind of drug testing performed on employees, the criteria for testing, and information about the type of drug testing done (urine, saliva, full panel etc). We know there are situations when elderly patients claim they have medications missing, and I'd llike to hear how others manage those situations. If you have a policy to share, it would be appreciated.
