I am director of Employee Health
Services for a system of 5000 plus employees, not including contract staff, volunteers or LIPs. 
Our system has a mandatory Flu vaccine policy that states proof of vaccine provided to EH , receive vaccine through Employee Health (EH) or file declination in EH, including medical/religious exemption requests by a set deadline annually. Those who decline are required to wear mask when w/i 6 ft of a patient once flu season has been declared by our system Infection Prevention  and Quality executive. 
Annually, EH creates and updates a flu module with the policy requirements, expectations, compliance, CDC recommendations, flu symptoms, CDC guidelines  for RTW after diagnosed with the Flu, CDC VIS  and masking guidelines. In addition, the module is followed b post test and an acknowledgment to the policy, the  requirements, compliance expectations and information sent to department managers. This is sent out via our education online portal. 

If an employee has a concern, they are referred to EH and our employee reparations team where each situation is reviewed. Our policy was reviewed by legal prior to establishing. 
EH is careful not share any medical information surrounding the decline of any vaccines. Flu vaccine declination lists are not posted for viewing by co-workers, other department Managers  or customers, as well as we do not post a sign for visitors stating yellow mask is an indicator staff has not received vaccine.
