Hello all

In light of changes to USP 800, medical surveillance has again come to the top opf the list at our institution. I am working with my team to revise our baseline and annual questionnaire. We do not do any lab work. Our questionnaire is a bit outdated with regard to reproductive questions as well as a targeted ROS.   Are anyone of you willing to share your most recent questionnaire that addresses the potential carcinogenic, mitogenic and reproductive effects of exposures.
