Yes, we have them start and hope they bring their documentation on their start date, if not, we used to do the 2 step Mantoux method and allowed them to start and took the chance they would follow up and have it read, which a certain amount of employees forgot, then had to do the second test later. When we started offering Tspot, then switched reference labs and now to Quantiferon Gold, we draw the tests on the date of hire and then have to wait about 3 days for results. Since we have been doing Quantiferon I have never had a positive, and when we did Tspot I had a few people whose results came back borderline or equivocal, and we had to retest or switch to Mantoux method and wait again for results.
Usually in a small Critical Access Hospital when we have a new hire its because they need them right away, so that is why we do this testing first. Our Drug screens are done offsite by 3rd party labs, and the offer is rescinded if anything were to show up positive, so we cannot ask them to have their bloodwork done until the job is officially offered and paperwork is signed.

As the employee health coordinator I have cleared new employees to begin work after TB testing results are known. We usually do IGRA testing at time of post offer screen. Does anyone allow new employees to begin work without knowing TB testing results, e.g. the IGRA is pending...
