Here is our scripting imbedded in the general consent for care the patient signs on admission:
In the event a healthcare worker is exposed to my blood or body fluid in a manner that may pose a risk for transmission of a blood- borne infection during this hospitalization, I am giving my consent to be tested for HIV. Testing for HIV, Hepatitis B and Hepatitis C will be done at no cost to me, so the healthcare worker can be treated promptly. I authorize release of this information to the exposed healthcare worker and his/her healthcare provider.

We have done the same, our general patient consent has an opt out of testing feature in the event someone is exposed to the patient's blood or body fluids.

We built the consent into our our admission process. This way if it is someone under anesthesia etc., we can initiate the process.
