Here is our scripting imbedded in the general consent for care the patient signs on admission:

In the event a healthcare worker is exposed to my blood or body fluid in a manner that may pose a risk for transmission of a blood- borne infection during this hospitalization, I am giving my consent to be tested for HIV.  Testing for HIV, Hepatitis B and Hepatitis C will be done at no cost to me, so the healthcare worker can be treated promptly.  I authorize release of this information to the exposed healthcare worker and his/her healthcare provider.

We built the consent into our our admission process.  Every patient that comes in is asked "In the event that a health care worker has a needlestick or other exposure to your blood or body fluid, do you consent to testing..."  This way if it is someone under anesthesia etc., we can initiate the process.
