[MCOH-EH] Titers and vaccinations

We require titers if record of recommended vaccines is not available.  We strongly recommend vaccination if immunity is inadequate by record/titer.  If vaccination is declined, there may be restrictions in placement (eg varicella and oncology or obstetrics).
