Hello Craig
We do a health history questionaire which includes more functional ability questions as well as the OSHA respiratory questionnaire. Hazardous drug, etc.
It is reviewed by the nurses and when there are medical issues assessed by the nurses, they bump it up to a provider especially with history of previous WC injury.

The provider will meet with the candidate.
The nurses also review the required immunizations.

The questionnaire is reviewed face to face or telephonically. The candidate must present for a drug test and tigers and a vision test.

If they are remote, they will get testing done through a national vendor,

We no longer have paper records. Once all paper work is completed we scan into our EME and discard the papers.

Be well
