Similar process for us. We have a formal diversion process, however, already in place that identifies specific behaviors (e.g., waste not done immediately, missed count). This program was championed by nursing.
We add an additional screening panel to our standard 9-Panel pre-employment. We use CRL.
P.S. No tests we have done for diversion have come up positive since this program began. We have terminated two people for multiple suspected diversions (diversion indicator 3 times, with or without positive
screen).

I would appreciate info on the following questions related to suspected fentanyl use:
· How would you proceed if an employee was suspected of fentanyl use?
It depends on how we become aware of the suspected usage. If it’s identified through our pharmacy surveillance reports, pharmacy sends the reports to the manager and notifies the manager of potential diversion. The manager then
conducts an investigation and contacts HR if diversion is in fact suspected. A huddle group is then convened with the department manager and/or director, pharmacy, human resources, and employee health to discuss the facts of the case and develop an action
plan. If testing is warranted, then the manager and human resources meet with the employee to take a statement and let the employee know that he/she will be drug tested. The employee is then brought to employee health for urine drug testing. After testing,
we arrange safe transportation to get the employee home. The employee may be suspended pending test results, which can take up to five days. (There are a lot of if/then steps in our process.)
· Would you test urine, saliva or blood?

It's a separate send out to our vendor and not part of our 10- panel used for pre-employment physicals or suspected impairment testing.
· Where would the specimen be sent for testing?
We utilize an outside vendor - Medtox. Our lab is looking into the possibility of testing in house.
