Attached is the form I use.   I adjust it for each job and each incident as they occur.

It’s not a plan … but it helps.

Hi Deb,

Specific to seizures, we found these resources on JAN(Job Accommodation Network): https://askjan.org/disabilities/Epilepsy-Seizure-Disorder.cfm and about halfway down the page is a link Sample Plan of Action (which is the template we are looking at using).

But we are hoping to cast a wider net and offer these tailored safety plans to more than just employees who have seizures.  And maybe include those who have mental illness and have noticeable symptoms that can, if caught early, can be directed to their provider and treated (and helped through the FMLA/ADA route, if needed).

I am also very interested – I have an employee who has seizure activity. His is a safety sensitive job.  We have sent the MD job descriptions, etc but the MD keeps returning him to full duty. The seizure activity episodes are growing closer together.  Hoping someone has a plan…

I’m very interested in this thread. Heather, if you receive any responses directly, would you mind forwarding to me at the contact info below? We recently had an employee returned from LOA after suffering a TBI and developing a seizure disorder. His physician kept releasing him to regular work saying “He could have a seizure anywhere so I’m not going to tell him he can’t work.” He wasn’t considering that this employee was a critical care nurse and having a seizure at the bedside could have disastrous consequences.

Hello!

Does anyone use safety plans for employees with chronic conditions, such as seizures, uncontrolled diabetes and mental health disorders?  For example, a written ‘plan of action’ is someone is having a seizure at work?

If so, do you have any resources, ect?

Thank you,

Heather
