Current guidance on Tdap is to vaccinate once per lifetime for adults, but to vaccinate pregnant women during each pregnancy.  However, we only offer it once to medical center staff for the protection of our patients.  Pregnant women are encouraged to get their boosters from the physician providing their obstetric care.

The way I understand it is and how we proceed is for employees in a direct patient care setting handling infants under 12 month, i.e. nursery, labor and delivery, RT, Radiology, lab technicians, ER, Urgent cares, family practice and pediatrician office. The recommendation is for ONE administration of Pertussis containing vaccine in an adult lifetime, 18 yrs and older. (TDAP) Also any pregnant women will receive TDAP in the second trimester.  We remember it and teach staff the P, is for Pertussis and for Pregnant and People holding babies, yes vaccinate ONCE.

 

Tetanus is still  recommended in employees 10 year interval secondary to injury with metal or burns. T is for Tough guys, that get hurt and don’t want to stop Talking…..(lockjaw) J yes vaccine q 10 yrs.

 

So the reason you are vaccinating is the driving factor. I hope this helps.

Our ID doc sent this out yesterday (see below) and said no recommendations from ACIP yet but can’t imagine there won’t be a change since most are already doing it.. 

 

We follow current ACIP recommendations for HCW; 1 dose of Tdap.  However if staff have a work related injury (including needlestick or sharps) and more than 5 years since last tetanus booster we give Tdap (only tetanus vaccine we have) per CDC recommendations.

FDA Approves Expanded Use Of Tdap Vaccine For Repeat Vaccination.

NJBIZ (NJ) (1/15, Vecchione) reports the Food and Drug Administration “approved the expanded use of [Sanofi’s] Adacel (Tetanus Toxoid, Reduced Diphtheria Toxoid and Acellular Pertussis (Tdap) Vaccine Adsorbed) to include repeat vaccination to help protect against tetanus, diphtheria and pertussis.” Sanofi provided results of a study on the effectiveness of the vaccine for repeat vaccinations in adults “to the Centers for Disease Control and Prevention’s Advisory Committee on Immunization Practices for their consideration in future recommendations.”

Hello-

 

Do any hospitals/med centers have policy on re-immunizing HCWs with Tdap sooner than the usual 10 year cycle? CDC doesn’t seem to indicate the usefulness, but our OB feels that the pertussis immunity wanes within 2 years, so we’re debating whether we need to consider a policy for re-immunizing, esp in OB and Peds.

 

Any evidence (literature or expert guidance) to back it up would be appreciated!
