Our process is similar to what Yvonne does, except that new hires are allowed to decline both vaccines and titers, but to do so, they have to sign a waiver indicating that they may be placed on Leave of Absence without pay if there is known
outbreak. Except flu, which requires the written medical or religious waiver.

At hire, we request proof of immunizations (varicella, MMR, Tdap, Influenza and Hepatitis B ( if in exposure prone position). If they do not have proof of vaccine, we draw titers for measles, mumps, rubella,
Varicella and Hepatitis B ( if in exposure prone position). Tdap vaccine is administered if they have no proof. Flu vaccine is administered during flu season (usually given August –May). If titers are negative,
they are notified to return for vaccines. If opposed to receiving vaccines or if they have a medical contraindication, they can submit a request for an exemption. The request is reviewed by a committee, and
a decision rendered.

We require documentation of MMR (2 doses) and varicella (2 doses). If no documentation of vaccines, then we do titers for measles, mumps, rubella, and varicella. If titers are negative, they are required to
get the vaccine.

For those of you working in hospital settings, which vaccines do you require proof of? (Do you take applicants’ word or do you require
proof)? What titers do you offer/require?
