We require documentation of MMR (2 doses) and varicella (2 doses).  If no documentation of vaccines, then we do titers for measles, mumps, rubella, and varicella.  If titers are negative, they are required to get the vaccine.

For those of you working in hospital settings, which vaccines do you require proof of?  (Do you take applicants’ word or do you require proof)?  What titers do you offer/require?
